Congratulations on completing the online coursework for the EMT program! Please take a few minutes to complete the following form so we can get your equipment kit ready to ship.
- Please enter the email address you have been using to communicate with us during your course. This will ensure your kit tracks to your account in our system properly.
- Verify your current shipping address (the location you want us to send the equipment kit)
- Click the Submit button
Once you submit your current address our staff will get your kit put together and shipped to you. You will receive a tracking number in an email so you can see the progress of it.
